Being naked (or at least almost naked) adds greatly to the feeling of helplessness and vulnerability. (For the same reason, a gag is essential too - I cannot even beg for mercy.)
Therefore, either naked or in (short!) shorts only - depends on the scenario I'm imagining.

I'm fantasizing about a strict hogtie, with my ankles crossed so that I cannot roll around, and my elbows tied too, so I can hardly move at all. Like any tieup, it wouldn't be complete without a gag.

I like the idea too - but I doubt I could endure the hanging variant for more than a few minutes (impossible to push myself up to get some relief for my wrists and shoulders!)
