Question:
WITHOUT tape (because I love a challenge) what is the perfect bandana gag without any chance of it coming out or slipping off your mouth and lips?
I have tried knotted bandana gags and a bandana over top of that one, but it always comes loose.

Cleave gags are more for aesthetics, than for functionality. They look good on TV and in the movies, and even on your friend, tied up on the bed or in a chair, but the reality, is that they can be pushed out with the tongue fairly easily.
Still, there's just something about a captive cleave gagged with a bandana, bandage, or neckerchief, especially an old square neckerchief.

Is there a limit to the number of bandannas? At least one as stuffing, then one tied as tightly as possible (double knotted in back) holding it in. Another on top of that one, again knotted in back. Repeat as necessary.
