What has been your worst gag?

I was doing a bit of self bondage, which for me is pretty much a good tight gag. I stuffed my mouth with my worn bikini briefs and wrapped tape around my mouth and head with more under my jaw. What my dumb ass forgot was that at the time, I had longer hair and sideburns. In my haste to shut myself up, I got my hair stuck in between the many layers of tape. It wouldn't come off. I soent half an hour in the shower trying to wet the tape to get it off. Lost a fair few hairs that day. Amateur hour at its finest.
