After years of feeling her life in the CIA wasn't too different from her life of crime, Sarah got a mission that changed her life forever for the good! Sarah becomes one of Chuck's government handlers, along with Colonel John Casey of the NSA. The three of them form the CIA-NSA joint black-ops team known as Operation Bartowski. She is assigned the task of protecting Chuck after he unintentionally uploads A machine that makes his brain work like a super computer known as The Intersect! From her first interaction with Chuck, she found herself liking him, and told him so, before they eventually became lovers!

They left the C.I.A and set up their own private Spy Agency where they could choose their own cases and help and support where they felt it was right to do so! They called their Agency the Carmichael agency after their work names Sarah and Charles Carmichael, (After they tied the knot and became married!) They pulled in trusted friends Morgan Crimes, John Casey and his new girlfriend and ex KGB agent Gertrude Verbanski, (Carrie-Ann Moss). She merged her renowned and even more successful private spy agency, The Verbanski Cooperation with the Carmichael agency. This made them the best in the game at what they do!

Gertrude had gone missing and evidence and records of previous jobs she had undertaken meant that the prime suspect was Nicole Wallace, (Olivia D'Arbo) The Queen Pin of crime! This was confirmed to Sarah, after she had a meeting with Jill Roberts, (Jordana Brewster) who had hired Gertrude to look into Nicole. Sarah had always been suspicious of Jill after she claimed to unknowingly have worked with the evil terrorist organisation Fulcrum! Chuck believed her and helped her get a job at the C.I.A, this greatly annoyed Sarah as she was her husbands ex-fiancé and had an history of using him! Still the evidence pointed too Nicole so Carmichael Agency had a plan to make!

Owing to Chuck's history with Jill he thought it would be best for the agency, and his Marriage! That he supported Casey who was worried greatly about Gertrude and Morgan should help him as he as a great history for screwing things up! That left Sarah who using her great skills in speed, agility and stealth breaking into the headquarters come home of Nicole was achieved with no great challenge to the master Spy! However no matter how skilled Sarah is as a spy, you don't get where Nicole as in life without been great at your job also! Before Sarah realised Nicole was waiting for her she recieved a flipping dragon kick, (Similar too the one Bruce Lee used on the guy who killed his sister on Enter the Dragon movie!) The assassination was investigated by Emily Prentiss, (Paget Brewster), and her JTF-12 team, Interpol criminal profilers! Izzy also presumably killed additional victims as an assassin. While in Chad in 2008, during a civil unrest occurring there at the time, Izzy met Mathew Downs, a U.S. Marine who suffered from PTSD and had established anarchist ideals against the U.S. government. The two became lovers and constructed an elaborate plan to attack Washington, D.C.

After making deadly test-runs of homemade bombs in N'Djamena, the capital city of Chad, killing thirty people, mostly at a church and a train station, Izzy then began conducting bank robberies all across the world in order to fund for her and Matthew's attack, killing a random hostage during each bank robbery. Arriving in the U.S. to continue the funding phase, Izzy recruited amateur bank robbers and brothers Chris and Oliver Stratton as her accomplices, reshaping them into efficient criminals. The trio began committing bank robberies in areas making a straight path to D.C. During these robberies, she continued to shoot a random hostage before fleeing. All the while, Matthew would operate behind-the-scenes, his involvement not being known to Chris and Oliver.

This time Izzy's mission didn't go to plan and not only was she captured by JJ after trying to kill her son Henry and her husband William LaMontagne! Her lover Matthew was shot dead by FBI agent Hotchner the lead agent of the criminal profiler team, The Behavioural Analysis Unit! While in prison she had no other plans than to get revenge when she was released! However Emily Prentiss had once been compromised by Nicole Wallace, (Olivia D'Arbo)! Compromised by Nicole meaning she got to near to her operations once so Nicole dealt with this in her special bondage, BDSM and Lesbian Control way!
